About The Position

Work involves providing for the assessment, care, treatment, and general welfare of patients receiving services on the Forensic Assertive Community Treatment (FACT) Team. May train others as well as delegate to Licensed Vocational Nurse (LVN). Works under moderate supervision, with limited latitude for the use of initiative and independent judgment.

Requirements

Associate’s degree in nursing from an accredited school of nursing.
Licensed to practice as a Registered nurse (RN) in the state of Texas
Currently licensed and in the good standing to practice by The State Board of Nursing.
A valid Texas driver’s license and good driving record are required along with a copy of a valid auto insurance card.
Must be able to bend, stoop, kneel, crouch, squat, lift 25 pounds, sit for an extended period, and reach overhead.

Nice To Haves

Bachelor’s in nursing preferred.
Two years’ experience as a Registered Nurse preferred, experience in psychiatric nursing is preferred.

Responsibilities

Under the supervision of the Director of Nursing, provides professional nursing services to FACT Team clients within the limits of Professional Licensure and provide assistance to other medical related services within the organization as needed.
Responsible for performing assessments on all clients, help keep assessments updated, developing nursing care plans, providing interventions, and evaluating provision of nursing care.
The nurse will be an integral part of helping coordinate medical health needs of clients.
Frequent travel within the region is required, including home visits and community-based services.
The nurse will participate in weekly meetings with team.
