About The Position
Pride in North Carolina, LLC is seeking a Facilitator for its High Fidelity Wraparound program. The Facilitator engages high-need youth and families in developing and monitoring individualized care plans and ensures that the principles and phases of the High-Fidelity Wraparound process are delivered with fidelity to the model. The role supports families in achieving positive outcomes through person-centered care, integrated plans, and natural support systems. Requirements
- Bachelor’s degree in human services or a related field; experience may substitute.
- Two years of experience providing services for children, youth, and families.
Responsibilities
- Engage youth and families in care planning and monitoring.
- Develop integrated plans and natural support systems.
- Maintain strong relationships with families and community partners.
- Ensure fidelity to High-Fidelity Wraparound principles.
- Document all activities accurately and in a timely manner.
