About The Position

The Evaluation Coordinator (EC) is a dual role Family Service Coordinator and Developmental Specialist (FSC/DS) position. The EC is primarily responsible for the implementation of Amplified Therapy, Inc.’s (Company) Intake Process, Evaluation, Assessment, Eligibility Process, and IFSP Process. The EC is the lead member of the Multidisciplinary Evaluation Team, is the hearing and vision screening professional, the IDA Lead Expert, and is responsible for ensuring that all quality assurance measures are followed within his/her job duties. The EC has marketing responsibilities in addition to other job duties assigned by the Company, within the Family Service Coordination (FSC) and Developmental Specialist (DS) role, as defined by the NM FIT Early Intervention program. The EC duties may include providing Family Service Coordination and/or Developmental Instruction to families/children, in accordance with the EI process as defined by the NM FIT Program regulations and standards. EC may also be required to participate in business operating duties.

Requirements

  • A bachelor’s degree in social work, counseling, psychology, nursing, special education, early childhood education, or any other closely related field.
  • A bachelor’s degree in another field plus two (2) years of direct experience in serving individuals with disabilities and/or families.
  • Certification as a Developmental Specialist II or III.
  • Certification as an Infant-Toddler Developmental Assessment (IDA) evaluator.
  • Training for family service coordination is in accordance with NM FIT Program standards within one (1) year of hire.
  • Completion of the web-based family­­­ service coordination training every three (3) years.
  • Receive reflective supervision at least once (1) every month.

Responsibilities

  • Act as the Lead Member of the Multidisciplinary Evaluation Team while performing Comprehensive Multidisciplinary Evaluations (CME’s).
  • Coordinate and execute all Annual Assessments within their assigned geographical location.
  • Fulfill the role of an IDA Lead.
  • Fulfill the role of a Developmental Specialist Supervisor and Trainer.
  • Participate in the Company Marketing Plan, including, but not limited to community outreach duties.
  • Attend organized community events, plan and complete meet-n-greets, and participate in child finds.
  • Perform a minimum volume of work for the compensation rendered.
  • Attend all IDA trainings offered at the State level, to become the IDA Lead expert for the Company.
  • Facilitate IDA trainings for Company’s staff members.
  • Act as the single point of contact for coordinating, facilitating and monitoring the delivery of services to ensure that services are provided in a timely manner.
  • Coordinate services across agency lines.
  • Assist parents in gaining access to, and coordinating the provision of EI services and other services as identified on the IFSP.
  • Explain to families about the EI services and their procedural safeguards.
  • Gather information from the family regarding their concerns, priorities and resources.
  • Coordinate the evaluation and assessment activities.
  • Facilitate the determination of the child’s eligibility.
  • Refer the family to providers for needed services and supports.
  • Schedule appointments for IFSP services for the child and their family.
  • Help families plan and prepare for their IFSP meeting.
  • Organize, facilitate and participate in IFSP meetings.
  • Arrange for and coordinate all services listed on the IFSP.
  • Conduct follow-up activities to determine that appropriate services are being provided.
  • Coordinate funding sources for services provided under the IFSP.
  • Monitor the delivery of the services listed on the IFSP.
  • Facilitate periodic reviews of the IFSP.
  • Ensure that a transition plan is developed at the appropriate time.
  • Work in a coaching role with the family or other caregiver, the design of learning environments and implementation of planned activities that promote the child’s healthy development and acquisition of skills that lead to achieving outcomes in the child’s Individualized Family Service Plan (IFSP).
  • Provide families and/or other caregivers with the information, skills, and support to enhance the child’s development.
  • Address all developmental areas: cognitive, communication, physical/motor (vision and hearing), social/emotional and adaptive development.
  • Supervise other DS’s as determined and assigned by the Company.
  • Conduct training of DS’s.
  • Develop, implement, manage and report on Individual Professional Development plans for all assigned DS’s.
  • Provide reflective and clinical supervision.
  • Communicate with Company’s management related to supervision, as required by the Company.
  • Become trained in the use of hearing and vision screening equipment and associated procedural protocols.
  • Perform all hearing and vision screening both in the comprehensive, multidisciplinary evaluation settings, as well any other community or individual settings required by the Company.
  • Ensure proper use, care and safekeeping of all equipment and ensuring required maintenance and repairs are reported immediately to the Company.
  • Confirm arrangements are made through the Company to facilitate necessary repairs and maintenance in a timely fashion.
  • Train others in the proper use of all hearing and vision screening equipment and procedural protocols.
  • Works towards fulfilling Company’s Mission, Vision, and Values.
  • Coordinates the intake, multidisciplinary evaluations and assessments, and determines eligibility.
  • Coordinates evaluations and assessments, eligibility, and development of the IFSP.
  • Selects/implements evidence-based practices which supports the evaluation and functional assessment process.
  • Facilitates and participates in multidisciplinary meetings to review evaluation results, integrate findings with other disciplines, offer recommendations, and develop individual treatment programs.
  • Participates in team discussions in an articulate, positive, and productive manner.
  • Participates in Company’s Professional Development Plan.
  • Completes all documentation in a timely manner.
  • Attends departmental/business staff development activities.
  • Maintains proficiency in family service coordination and evaluation and assessment by attending workshops, conferences, and trainings.
  • Attends all mandatory in-services and participates in Company committees as requested by supervisor.
  • May represent the Company while participating in community outreach events, activities, and meetings.
  • Will participate in the Company Marketing Plan that includes community outreach efforts, educational presentations, community collaborations, and any other undefined marketing duties deemed beneficial by the Company.
  • Responds to emergency situations in a competent manner.
  • Maintains equipment in proper working order.
  • Adheres to established Company guidelines within the Company’s Official Documents.
  • Assists in quality management, research, public relations, and program development projects when appropriate.
  • Maintains assigned equipment in proper working order.
  • Perform routine repairs.
  • Perform other duties as assigned.

  • It shall be at the Company’s Leadership Teams discretion, to determine and re-distribute this position’s job duties between that of a Family Service Coordinator, Developmental Specialist and Evaluation Coordinator, to best meet the needs of the Company at any given time.
