About The Position

The Data & Evaluation Coordinator plays a central role in helping SOKS understand, manage, improve, and communicate the impact of its rural health initiatives. By developing strong data systems, performance dashboards, and meaningful reporting, this position ensures SOKS has the information needed to meet KDHE requirements, strengthen partnerships, guide organizational decision-making, improve program effectiveness, and clearly demonstrate measurable outcomes for individuals with intellectual and developmental disabilities, families, healthcare providers, schools, and rural communities. Ultimately, this role helps transform data into insight, insight into action, and action into measurable impact.

Requirements

  • Bachelor's degree in Public Health, Health Administration, Healthcare Management, Nonprofit Management, Human Services, Social Work, Education, Public Administration, Business Administration, Data Analytics, Statistics, or a related field required.
  • Proven experience with data tracking, program evaluation, reporting, or grant compliance.
  • Experience with dashboard creation, KPI reporting, and performance measurement.
  • Experience utilizing data management and reporting tools such as database tools, Excel, Google Sheets, Power BI, Tableau, or similar platforms.
  • Valid driver's license and ability to be covered under the organization's insurance policy
  • Self-initiating and comfortable building systems from the ground up.
  • Highly detail-oriented and committed to data accuracy.
  • Creative in developing practical tracking and reporting solutions.
  • Excellent written and verbal communicators.
  • Comfortable working across multiple programs, teams, and external partners.
  • Able to translate complex information into meaningful insights for diverse audiences.

  • Knowledge of health, disability services, nonprofit, education, or public-sector programs and operations.
  • Skill in data quality review, validation, and documentation to ensure accuracy, integrity, and compliance.
  • Skill in coordinating cross-functional projects and collaborating effectively with diverse stakeholders.
  • Experience preparing reports and supporting reporting requirements for funders, government agencies, or other external partners.
  • Passionate about helping tell the story of SOKS' impact through accurate, meaningful, and compelling data.

Responsibilities

  • Compile all data required for quarterly, annual, and ad hoc reports to the Kansas Department of Health and Environment (KDHE).
  • Track grant baseline values, targets, milestones, progress updates, and required deliverables.
  • Create and maintain data tracking tools, repositories, dashboards, reporting templates, and supporting documentation.
  • Establish data quality standards, validation processes, and routine quality assurance reviews.
  • Identify, investigate, and communicate suspected data quality issues.
  • Support continuous performance improvement by producing timely, meaningful, and actionable reporting.
  • Assist in maintaining project issue logs, risk logs, and performance tracking documentation.
  • Collect, analyze, validate, and maintain data necessary to support federal, state, and grant-funded program reporting requirements.
  • Monitor grant performance metrics, key performance indicators (KPIs), and program outcomes to measure progress toward established goals and objectives.
  • Prepare accurate and timely reports, dashboards, and supporting documentation for funding agencies, leadership, and other stakeholders in accordance with grant requirements.
  • Ensure data integrity through ongoing quality assurance, reconciliation, and validation of program data and supporting records.
  • Track progress toward grant deliverables, performance targets, and reporting milestones, identifying potential risks or deficiencies and recommending corrective actions as needed.
  • Collaborate with internal staff, program leaders, and external partners to collect required information and ensure compliance with grant reporting standards.
  • Maintain documentation supporting grant activities, expenditures, performance measures, and audit readiness.
  • Support continuous improvement efforts by analyzing trends, evaluating program effectiveness, and providing data-informed recommendations to enhance grant outcomes.
  • Perform other grant reporting, compliance, and performance measurement responsibilities as assigned to support organizational and funding agency objectives.
  • Create KPI dashboards and reporting views for leadership, program teams, rural coordinators, referral coordination services, and healthcare hubs.
  • Develop activity tracking mechanisms that enable regional coordinators to demonstrate progress toward grant objectives before formal outcome milestones are achieved.
  • Implement reporting processes and technology supporting youth programming, preventative health education, inclusive health training, Healthy Athletes screenings, referral follow-up, and other health initiatives.
  • Design and implement efficient data collection workflows for Healthy Athletes screenings and referral follow-up activities.
  • Configure reporting systems supporting Referral Coordination Services.
  • Build flexible tracking tools that can evolve alongside program and grant requirements.
  • Develop reporting tools for both the statewide healthcare hub strategy and individual hub performance.
  • Help establish reporting expectations and data exchange requirements with healthcare hubs.
  • Collaborate with healthcare hub counterparts regarding ongoing reporting and contractual data exchange needs.
  • Collaborate with Referral Coordination Services regarding contractual reporting and data exchange requirements.
  • Track referral service performance, healthcare provider engagement, screening outcomes, referral outcomes, and healthcare utilization, where applicable.
