About The Position

The Evaluation Coordinator (EC) is a dual-role position encompassing both Family Service Coordinator (FSC) and Developmental Specialist (DS) duties. This role is primarily responsible for the implementation of Amplified Therapy, Inc.’s (Company) Intake Process, Evaluation, Assessment, Eligibility Process, and IFSP Process. The EC serves as the lead member of the Multidisciplinary Evaluation Team, acts as the hearing and vision screening professional, and is the IDA Lead Expert. This position ensures all quality assurance measures are followed within its job duties and has marketing responsibilities in addition to other duties assigned by the Company, within the FSC and DS role as defined by the NM FIT Early Intervention program. The EC's duties may include providing Family Service Coordination and/or Developmental Instruction to families/children, in accordance with the EI process as defined by the NM FIT Program regulations and standards. The EC may also be required to participate in business operating duties.

Requirements

  • A bachelor’s degree in social work, counseling, psychology, nursing, special education, early childhood education, or any other closely related field.
  • A bachelor’s degree in another field plus two (2) years of direct experience in serving individuals with disabilities and/or families.
  • Certification as a Developmental Specialist II or III.
  • Certification as an Infant-Toddler Developmental Assessment (IDA) evaluator.
  • Training for family service coordination in accordance with NM FIT Program standards within one (1) year of hire.
  • Completion of the web-based family service coordination training every three (3) years.
  • Receive reflective supervision at least once (1) every month.

Responsibilities

  • Act as the Lead Member of the Multidisciplinary Evaluation Team while performing Comprehensive Multidisciplinary Evaluations (CME’s).
  • Coordinate and execute all Annual Assessments within their assigned geographical location.
  • Fulfill the role of an IDA Lead, a Developmental Specialist Supervisor and Trainer.
  • Participate in the Company Marketing Plan, including community outreach duties such as attending organized community events, planning and completing meet-n-greets, and participation in child finds.
  • Attend all IDA trainings offered at the State level to become the IDA Lead expert for the Company.
  • Facilitate IDA trainings for Company’s staff members in conjunction with the Company’s Vice President and EDEI.
  • Provide FSC services as designated in the IFSP, acting as the single point of contact for coordinating, facilitating, and monitoring the delivery of services.
  • Coordinate services across agency lines.
  • Assist parents in gaining access to, and coordinating the provision of EI services and other services as identified on the IFSP.
  • Explain EI services and procedural safeguards to families.
  • Gather information from the family regarding their concerns, priorities, and resources.
  • Coordinate evaluation and assessment activities.
  • Facilitate the determination of the child’s eligibility.
  • Refer the family to providers for needed services and supports.
  • Schedule appointments for IFSP services for the child and their family.
  • Help families plan and prepare for their IFSP meeting.
  • Organize, facilitate, and participate in IFSP meetings.
  • Arrange for and coordinate all services listed on the IFSP.
  • Conduct follow-up activities to determine that appropriate services are being provided.
  • Coordinate funding sources for services provided under the IFSP.
  • Monitor the delivery of the services listed on the IFSP.
  • Facilitate periodic reviews of the IFSP.
  • Ensure that a transition plan is developed at the appropriate time.
  • Provide Developmental Instruction (DI) services in a coaching role with the family or other caregiver, designing learning environments and implementing planned activities that promote the child’s healthy development and acquisition of skills.
  • Supervise other DS’s as determined and assigned by the Company, including training, development, implementation, management and reporting of Individual Professional Development plans, reflective and clinical supervision, and communication with Company’s management.
  • Become trained in the use of hearing and vision screening equipment and associated procedural protocols.
  • Perform all hearing and vision screenings in comprehensive, multidisciplinary evaluation settings, as well as any other community or individual settings required by the Company.
  • Ensure proper use, care, and safekeeping of all screening equipment and report any required maintenance and repairs immediately to the Company.
  • Train others in the proper use of all hearing and vision screening equipment and procedural protocols.
  • Coordinate the intake, multidisciplinary evaluations and assessments, and determine eligibility.
  • Coordinate evaluations and assessments, eligibility, and development of the IFSP.
  • Select/implement evidence-based practices which support the evaluation and functional assessment process.
  • Facilitate and participate in multidisciplinary meetings to review evaluation results, integrate findings with other disciplines, offer recommendations, and develop individual treatment programs.
  • Participate in team discussions in an articulate, positive, and productive manner.
  • Participate in Company’s Professional Development Plan.
  • Complete all documentation in a timely manner.
  • Attend departmental/business staff development activities.
  • Maintain proficiency in family service coordination and evaluation and assessment by attending workshops, conferences, and trainings.
  • Attend all mandatory in-services and participate in Company committees as requested by supervisor.
  • Represent the Company while participating in community outreach events, activities, and meetings.
  • Participate in the Company Marketing Plan that includes community outreach efforts, educational presentations, community collaborations, and any other undefined marketing duties deemed beneficial by the Company.
  • Respond to emergency situations in a competent manner.
  • Maintain equipment in proper working order.
  • Adhere to established Company guidelines within the Company’s Official Documents.
  • Assist in quality management, research, public relations, and program development projects when appropriate.
  • Perform routine repairs on assigned equipment.
  • Perform other duties as assigned.
