The Brook Hospital KMI, located at 8521 La Grange Rd, is seeking a Full-Time Evaluation and Referral Coordinator/Intake Coordinator to work Monday-Thursday, 1 PM - 9:30 PM.

Requirements

• Master's Degree from an accredited school in the Behavioral Sciences.
• Ability to work with minimal supervision.

Responsibilities

• Answer all incoming intake calls and provide appropriate resolution.
• Conduct initial evaluations to determine an appropriate treatment level of care.
• Complete admission paperwork, including admission data entry.
• Work with minimal supervision.
• Work with all hospital units, business office, medical staff, medical records, utilization review, marketing, and nursing.
