ER Discharge Coordinator PRN
About The Position
The ER Discharge Coordinator is responsible for point-of-care patient bedside scheduling as part of the organization’s centralized ER and Urgent Care follow-up programs. The role focuses on initiating the patient experience at the bedside to increase contact rates, enable timely scheduling, and improve the overall patient experience. The coordinator contributes to a positive, patient-centered, quality-driven environment and provides service to patients, customers, stakeholders, and colleagues. This is a PRN (as-needed) position, scheduled Saturday and Sunday from 11:00 a.m. to 7:30 p.m., with additional shifts as needed during the week. One year of related experience is preferred. Responsibilities
- Speak with customers, patients, and stakeholders in a pleasant and professional manner.
- Communicate appropriately and clearly with department leadership, coworkers, and customers or patients.
- Schedule patients for follow-up care at the bedside when appropriate.
- Ensure performance and quality standards are met.
- Provide superior customer service to internal and external customers and patients.
- Support the organization’s vision and strategy, promote its services, and elevate issues to leadership as appropriate.
- Suggest ideas for positive changes to department policies and procedures.
