About The Position

The Program works in collaboration with MassHealth to address the immediate daily necessities that arise from the inequities caused by the social determinants of health. The Enrollment and Care Coordination Specialist will screen individuals for CSP eligibility and will complete enrollment for individuals in the program including orientation to services, service agreements, consents, and data entry into the electronic health record software for each member. They also provide training, monitoring, feedback and corrective action to housing specialists and assign and track housing specialists caseloads in collaboration with the team lead, as needed.

Requirements

  • High School Diploma, GED or equivalent, required.
  • Minimum of two years’ experience with housing and behavioral health population, required.
  • Valid driver’s license and acceptable driving history, required.
  • Must be able to collaborate with a multi-disciplinary team, possess excellent written and verbal communication skills with attention to detail, required.

Nice To Haves

  • Experience with electronic health records, billing and/or referral management are strongly preferred.

Responsibilities

  • Screen individuals for CSP eligibility and complete enrollment.
  • Provide orientation to services, service agreements, and consents.
  • Perform data entry into the electronic health record software for each member.
  • Provide training, monitoring, feedback, and corrective action to housing specialists.
  • Assign and track housing specialists caseloads in collaboration with the team lead, as needed.
  • Outreach to community providers to develop relationships and develop referral pathways.
  • Verify insurance prior to enrollment and monitor insurance eligibility for each member.
  • Obtain authorizations from the insurance provider for each member.
  • Track authorization expiration and obtain new authorizations.
  • Participate in live review meetings with insurance company representatives for each member.
