The Encounter Specialist participates in daily Care Team Huddles to ensure collaborative care is given to each patient on every visit. This role performs a review of patient's medical records for upcoming, scheduled clinic visits, abstracting pertinent data related to activity within the healthcare system since the last visit, quality indicators, and the patient's personal clinical goals of therapy. The specialist makes notes as necessary to serve as a reminder of issues that need to be addressed during the clinic visit. They greet and escort patients to the exam room, obtain and document vital signs, and assess medication regimens in detail. The Encounter Specialist also engages patients in conversation regarding their reason for visit and overall health, assesses social environment, gaps in care, quality measures, and progression/barriers to therapy goals. They prepare charts with labs or other data for the provider, remain in the exam room to enter all findings, provider notes, orders, and plan of care instructions. Additionally, they provide instructions and educational materials to patients, inform them about the timing of their next visit, and document all activities in the electronic medical record. Other responsibilities may include checking schedules, organizing patient flow, assisting patients, answering calls, sending/receiving medical records, facilitating referrals, completing forms, scheduling appointments, insurance verification, and pre-certification activities. The role also involves enhancing professional growth through in-service meetings and educational programs, adhering to controlled substance regulations, practicing universal precautions, and completing training related to HCC coding.
