The Encounter Specialist will participate in daily Care Team Huddles to ensure collaborative care for each patient. This role involves reviewing patient medical records for upcoming visits, abstracting pertinent data, and making notes for discussion during the visit. The specialist will greet and escort patients, obtain vital signs, assess medication regimens, and reconcile allergies. They will engage patients in conversation about their health, assess social environments, gaps in care, and progress towards goals. The specialist will prepare charts, document all findings in the electronic medical record, and provide patient instructions and educational materials. Additional responsibilities include checking schedules, organizing patient flow, assisting patients, and fulfilling clerical duties such as sending/receiving records, facilitating referrals, completing forms, scheduling appointments, and insurance verification. Professional growth is encouraged through in-service meetings and educational programs. The role also requires adherence to controlled substance regulations and universal precautions.
