About The Position
The Young Adult Treatment Team (YACTT) is a multidisciplinary team dedicated to assisting young adults in developing the skills necessary for independent and productive lives. The team offers skill-building in areas such as life skills, coping mechanisms, symptom management, interpersonal relationships, and vocational/educational goals. YACTT employs assertive engagement strategies to overcome treatment barriers and meets clients 'where they are at,' with outreach being the primary service delivery method. The team emphasizes collaboration, holding regular meetings for care coordination and mutual support, and staff receive individual supervision from the Program Director, a Licensed Mental Health professional specializing in the young adult population. Recognizing that many clients have experienced significant trauma, the team operates with a trauma-informed care approach.
Requirements
- Bachelors in human services, rehabilitation, or occupational therapy.
- Minimum 2 years experience working with individuals with substance abuse and/or co-occurring mental health issues.
- Minimum 6 months experience in employment/education and/or psychosocial rehabilitation.
- Valid driver's license.

- Advocacy Unlimited: Recovery Support Certification recommended for individuals who identify themselves as having a mental illness or substance abuse.
Responsibilities
- Conducts assessments to identify employment and educational needs.
- Collaborates with individuals to create individualized, person-centered treatment plans and documents client progress.
- Uses motivational interventions and stage-wise interventions to assist clients in achieving their goals and progressing in their recovery.
- Establishes rapport with community employers to develop a network for employment opportunities.
- Maintains documentation in compliance with agency and program standards.
- Participates in staff meetings, supervision, trainings, and other assigned meetings.
- Supports individuals in linking with and engaging natural (family, friends) or community supports.
- Develops crisis plans for clients and collaborates with staff to respond to crisis situations.
- Provides skills teaching, including assistance with obtaining/maintaining housing and utilities, using transportation, meal planning/preparation, community safety, childcare, family care, and problem-solving.
- Provides intensive skill-building interventions and support for self-management, self-advocacy, health and well-being, relapse prevention, and community resource utilization.
- Participates in family education and support activities.
