About The Position
Care Lync is hiring a full-time Eligibility Specialist/Case Manager to provide activities that meet the needs of clients assigned to a consolidated care team. The role involves assessing participants’ disabilities and related needs, helping them achieve their goals, and creating plans tailored to each participant. Plans are modified annually or when a significant medical or social change occurs. The Case Manager coordinates with individuals and entities to support seamless delivery of physical, behavioral, and support services. The position also involves developing and maintaining professional relationships with participants and their families. Care Lync is a Nevada social services agency supporting people with physical disabilities who are enrolled in Medicaid waiver programs.
