About The Position

Deliver direct care services, care coordination, caregiver coaching, and Social Determinants of Health (SDOH) linkage for families of children 0–6 enrolled in CCBHC early childhood services. Work in conjunction with the Early Childhood Therapist to implement the plan of care, reinforce skills in natural settings, and support rapid access and crisis follow‑up.

Requirements

  • Bachelor’s in Psychology, Human Services, Early Childhood, or related field; or be equivalently qualified through a combination of related work experience and education, with one year of experience substituting for one year of education.
  • Ability to work professionally and in conjunction with other mental health center staff.
  • Completion of all required background checks, alcohol/drug screening process, and motor vehicle record check.

Nice To Haves

  • Experience with Kansas early childhood systems (Part C, Head Start) and school IEP/IFSP supports.
  • Experience working with children and families.
  • Experience and ability to work with children with a severe emotional disturbance, and the ability to react professionally and effectively in a variety of human service situations.
  • Prior experience should be in mental health or human services field.

Responsibilities

  • Provide quality, strengths based case management services to SED children and families.
  • Assure that referrals for case management services are acted upon timely, typically within 3 business days of referral.
  • Rapid engagement; barrier solving (transportation, childcare); appointment coordination.
  • Daily alignment with Therapist to implement interventions and match Level of Care.
  • Coordinate with Part C, pediatrics, early education/Head Start, child welfare, courts, Health Department, Healthy Families, Parents as Teachers, preschools, childcare centers, SUD/medical providers.
  • In‑home caregiver coaching on routines, co‑regulation, safety plans, and behavior supports.
  • SDOH screening and linkage (housing/food/transportation/benefits).
  • Documentation (CSRs, service notes, strengths and needs assessments) and assessments (CBCLs, TRFs, Caregiver/TRFs) aligned to CCBHC timelines and quality measures.
  • Meet client engagement expectations.
  • Ability to work a flexible schedule, as needed, to meet client/family needs.
  • Complete training requirements as necessary for provision case management and additional Rehabilitation Services, as determined by supervisor and KDADS.
  • Participate in coverage for afterhours/weekends on call rotation.
  • Other duties, as assigned.
