About The Position

The PACE Driver provides safe, timely transportation to participants and communicates information relevant to their plan of care with the interdisciplinary team (IDT). The driver serves frail elderly participants according to assigned responsibilities and PACE Program standards, and interacts directly with caregivers and contracted partners while conducting business for the Franciscan Senior Health and Wellness (FSHW) PACE program. PACE provides seniors and their families with care, nutrition, rehabilitation, transportation, and supportive services to help participants remain healthy and live in their own homes. The role reports directly to the PACE Transport Supervisor. Practice hours are Monday through Friday, 8:00 a.m. to 5:00 p.m., with no weekends, evenings, or holidays. Travel is required up to 75%. Candidates must be over age 23 for insurability requirements.

Requirements

High school diploma or GED.
One year of experience with a frail or elderly population.
Driver's license.
Auto insurance.
American Heart Association Cardiopulmonary Resuscitation (CPR) certification.
Must be over age 23 for insurability requirements.

Nice To Haves

One year of experience in a PACE program.
One year of transportation experience.

Responsibilities

Escort and transport participants in a timely and safe manner.
Inspect and maintain vehicle supplies and equipment, and ensure vehicles are in proper working condition.
Assist passengers on and off the vehicle safely and courteously.
Follow established schedules, adjust schedules for unanticipated changes, and use the safest and most efficient pickup and delivery routes.
Participate in participant care planning and contribute notations to the medical record.
Report daily to appropriate Franciscan Alliance PACE staff important information about changes, progress, or unusual events concerning participants.
Conduct daily checks of PACE vehicles before transporting participants.
