About The Position

Integrated Home Care Services (IHCS) provides Durable Medical Equipment (DME), Respiratory, Home Health, and Home Infusion services through a network of certified and accredited providers. The DME Transition Specialist processes patient referrals, maintains client information in a database, and provides clerical and administrative support to DME Customer Service and day-to-day care management activities. The role also provides telephone support for incoming and outgoing calls involving patients, providers, Transition Coaches, and other team members related to the Transition Care program, and maintains and expands the community resource database.

Requirements

• 1–2 years of customer service support experience in the healthcare industry; Home Health Services experience is preferred.
• Effective verbal and written communication skills.
• Ability to use various computer programs and applications, such as EMR/EHR systems.
• Ability to self-motivate and work independently.
• Customer focus and adaptability to different personality types.
• Ability to multitask, set priorities, and manage time effectively.
• Familiarity with patient referrals.
