About The Position

This work is providing Prearrest Delinquency Citation (PDC) Case Manager for the circuit and is responsible for ensuring that the program area complies with all laws, rules and policies related to youth referrals and intake process. This position may require flexibility with the work schedule.

Requirements

  • A high school diploma or its equivalent.
  • Experience using Microsoft programs to perform routine clerical and administrative duties.
  • Knowledge of case management practices.
  • Knowledge of interviewing and counseling techniques.
  • Ability to provide counseling and guidance to others.
  • Ability to make recommendations concerning the processing, handling, and disposition of delinquent youth.
  • Ability to investigate and complete inquiries and reports.
  • Ability to develop case plans.
  • Ability to coordinate case assignments for multi-problem clients.
  • Ability to plan, organize, and coordinate work assignments.
  • Ability to determine work priorities, assign work, and ensure proper completion of work assignments.

Nice To Haves

  • Two years of work experience. College education from an accredited institution can substitute at the rate of 30 semester or 45 quarter hours for each year of the required experience.
  • Experience providing support to a work unit/group.
  • A valid driver's license.

Responsibilities

  • Serve as the Prearrest Delinquency Citation (PDC) Case Manager for the circuit, ensuring program area compliance with all laws, rules, and policies related to youth referrals and intake processes.
  • Attend all required staff and departmental trainings and meetings, completing a minimum of 24 hours of training annually.
  • Operate general office equipment such as copy machines, typewriters, personal computer terminals, and fax machines.
  • Provide customer service to internal and external customers.
  • Confirm youth eligibility upon referral from the Florida Department of Juvenile Justice (FDJJ) and notify the referring law enforcement agency or state attorney’s office if non-eligible, or enter required data in the Juvenile Justice Information system (JJIS) Prevention Web if eligible.
  • Initiate contact with the youth’s family upon eligibility to schedule intake assessments.
  • Conduct in-person intake assessments to provide an overview of the program and complete the Prevention Assessment Tool (PAT).
  • Formulate a case plan using approved instruments that addresses the specific needs of the youth and family, negotiating, implementing, and modifying the plan based on input from the youth and family, and identified risks and needs.
  • Assist the youth and family with accessing community resources and services, including intra-agency services, education, and employment resources, to address needs and complete case plan requirements.
  • Work with the youth to establish personal goals and provide ongoing support to help them achieve their goals and maintain skills.
  • Discuss projected timeframes for face-to-face contact with youth and family to review documentation and determine if all plan requirements have been completed.
  • Review documents (e.g., community service hours, restitution/fees receipts, verification of participation in educational/CTE classes, counseling/treatment verification) to determine satisfactory completion of program requirements.
  • Follow up with supervisor to determine satisfaction in completing all requirements and create an extended case plan with deadline dates if requirements are not met.
  • Follow the circuit’s procedure upon completion of the case plan and notify the referring law enforcement agency and/or state attorney’s office of the youth’s compliance, proceeding with case closure.
  • Provide technical assistance and conduct case reviews with staff to determine appropriate case management intervention strategies.
  • Enter all case management activities into the Prevention Case Notebook Module, including intake assessment, case planning, check-ins, supervisory reviews, case staffing, and communication with referring agencies.
  • Upload copies of the case plan, documentation provided by youth and parents/guardians, and all correspondence to the JJIS Case Notebook Module.
  • Attend and participate in professional development trainings and meetings.
  • Follow all departmental policies, procedures, and administrative rules.
  • Provide assistance to other staff when language interpretation is needed.
