About The Position
The Discharge Planner is responsible for ensuring a smooth, safe, and effective transition of patients from inpatient behavioral health treatment back into the community. This role bridges the gap between clinical care and aftercare, coordinating services to maintain continuity of treatment and reduce readmission risk.
Requirements
- Ability to pass a Level 2 Background Investigation.
- Bachelor’s Degree in social work or related field required.
- Handle with Care, Verbal Deescalation and BLS/CPR required (May be obtained upon hire)
- Effective Oral and Written Communication Skills
- Must be available to attend the full hospital and departmental orientations
Nice To Haves
- Two (2) years' experience working in a psychiatric setting preferred.
Responsibilities
- Arrange and follow up on post-discharge services such as therapy, medication management, housing, and community support
- Meet with patients to review the discharge plan and ensure patient understanding
- Ensuring patients leave with prescribed medications and refills, and understanding how to access them
- Involving patients and their families in planning, providing education, and addressing questions or concerns in understandable language
