About The Position

The Discharge Planner works in the inpatient rehabilitation/post-acute care unit at Mosaic Life Care St Joseph Medical Center ARU in St. Joseph, Missouri. The role supports discharge planning and follow-up care for patients recovering from debilitating injuries or illnesses in a fast-paced hospital-based rehabilitation environment. The planner coordinates with patients, families, caregivers, and healthcare staff; identifies and arranges services and resources; maintains required forms; and participates in case reviews and group activities.

Requirements

• A bachelor's degree is preferred, or three years of experience.
• Handle With Care certification is required within 30 days of hire.
• Critical thinking skills and decisive judgment.
• Ability to work with minimal supervision.
• Ability to work in a stressful environment and take appropriate action.

Nice To Haves

• Master's degree preferred.

Responsibilities

• Communicate routinely with patients, families, medical staff, nursing staff, and other caregivers when arranging follow-up care.
• Keep track of Medicare forms and consent forms.
• Assist with discharge planning and processes.
• Identify appropriate resources, including transportation, housing, healthcare, and social or spiritual services, and provide referrals as part of the discharge plan.
• Participate in case reviews to evaluate case management and patient progress.
• Consult with healthcare team members to promote, monitor, and evaluate compliance with the patient's treatment plan.
• Interview and assess patients and/or their families, caregivers, and/or legal representatives.
• Determine, prioritize, provide, and/or arrange needed internal and external services and interventions.
• Assist with group activities.
