About The Position

The Discharge Planner coordinates activities of the Case Management Department under the direction of an assigned Case Manager or Social Worker and assists with developing and implementing discharge plans. The role supports case management referrals and authorizations, maintains and coordinates Medicaid state submission forms for review and filing according to facility procedures, and assists with discharge planning by interviewing patients and families and gathering information to develop safe, patient-focused discharge plans.

Requirements

• Knowledge of discharge planning, utilization management, and case management terminology and functions in both managed care and non-managed care environments.
• Basic to intermediate computer skills and knowledge.
• High school diploma.

Nice To Haves

• Current BCLS certificate upon hire and maintained current; preferred.
• Associate degree or LVN license preferred.
• Hospital-setting experience preferred.

Responsibilities
