DISCHARGE PLANNER
About The Position
Valley Hospital is seeking a dynamic and talented Discharge Planner to join their team. This is a full-time position, Monday through Friday, 8 AM to 5 PM. The Discharge Planner works with the Utilization Review department on certification and recertification of insurance benefits throughout the patient’s stay. They assist the treatment team in understanding barriers to discharge planning and must have a thorough understanding of the patient’s treatment through communication with the treatment team. The Discharge Planner advocates for the patient’s access to services during treatment team meetings and through individual physician contact. The Discharge Planner will carry a caseload, meet with patients daily, be responsible for completing referrals, family sessions, staffing’s, and coordinating with outpatient providers. Valley Hospital is a private 122-bed psychiatric hospital specializing in mental health care and chemical dependency counseling services, located centrally with easy access to greater Phoenix.
Requirements
- Master’s degree from an accredited college or university in social work, mental health, nursing, or related degree.
- Two (2) years of behavioral health experience required.
- A minimum of two (2) years’ experience in a healthcare setting or managed care company.
- Experience in patient assessment, treatment planning, utilization management, and/or case management.
- L.M.S.W., LAC, LCSW, LPC, LISAC or other healthcare related licensure required.
Responsibilities
