About The Position
The Discharge Planner assumes responsibility for the overall resource needs of the patient. This includes discharge planning, placement of the patient, aftercare needs, advocacy needs, housing referrals, and entitlement needs.
Requirements
- Bachelor’s degree from an accredited college or university in Social Work or related mental health field required.
- A minimum of two (2) years direct clinical experience in a psychiatric or mental health setting and direct experience in patient/family assessment required.
- Level 1 Arizona Fingerprint Clearance card or be able to obtain within 30 days of hire.
