About The Position

The Discharge Planner works in the inpatient rehabilitation/post-acute care unit at Mosaic Life Care St. Joseph Medical Center in St. Joseph, Missouri. The role supports patients recovering from debilitating injuries or illnesses by coordinating discharge planning and follow-up care with patients, families, caregivers, and healthcare staff. The unit is described as fast-paced, with opportunities to learn and grow.

Requirements

• Bachelor’s degree preferred, or 3 years of experience.
• Handle With Care certification within 30 days of hire.
• Critical thinking skills and decisive judgment.
• Ability to work with minimal supervision.
• Ability to work in a stressful environment and take appropriate action.

Nice To Haves

• Master’s degree preferred.

Responsibilities

• Communicate routinely with patients, families, medical staff, nursing staff, and other caregivers when arranging follow-up care.
• Track Medicare forms and consent forms.
• Assist with discharge planning and discharge processes.
• Identify appropriate resources—including transportation, housing, healthcare, and social or spiritual services—and provide referrals as part of discharge plans.
• Participate in case reviews to evaluate case management and patient progress.
• Consult with healthcare team members to promote, monitor, and evaluate compliance with patients’ treatment plans.
• Interview and assess patients and/or their families, caregivers, or legal representatives.
• Determine, prioritize, provide, and/or arrange needed internal and external services and interventions.
• Assist with group activities.
