About The Position

The Discharge Planner coordinates all discharge needs for hospital discharges. This role works collaboratively with the Social Worker and the Case Manager covering the clinical area. The Discharge Planner processes physician orders based on patient choice and insurance contracts, coordinates home health, durable medical equipment, orthotics, and infusion services. They utilize an electronic referral system, ensure follow-up with companies for benefits and services, and obtain necessary forms for completion. The role also involves assisting the social worker with patient choice, reviewing medical records for discharge needs, and communicating case finalization to staff. Adherence to University and unit-level policies and procedures is required.

Requirements

  • High School Diploma or equivalent/relevant experience, certification or license
  • Minimum 1 year of relevant experience required
  • Refer to department description for applicable certification requirements
  • Learning Agility: Ability to learn new procedures, technologies, and protocols, and adapt to changing priorities and work demands.
  • Teamwork: Ability to work collaboratively with others and contribute to a team environment.
  • Technical Proficiency: Skilled in using office software, technology, and relevant computer applications.
  • Communication: Strong and clear written and verbal communication skills for interacting with colleagues and stakeholders.

Responsibilities

  • Processes the physician order based on the patient’s choice and based on insurance contract agreements with certified agencies.
  • Coordinates all Home Health, Durable Medical Equipment, Orthotics and Infusion.
  • Utilizes electronic referral system to send referrals for all post discharge needs to licensed providers.
  • Assures follow-up with companies for the coordination of benefits and services.
  • Obtains any required forms for completion with the physician attending, qualified practitioner or ordering physician for completion.
  • Assists the social worker with obtaining patient choice when warranted.
  • Reviews Medical Record electronically to obtain appropriate documentation for post discharge needs and coordinate services.
  • Communicates the finalization, delivery, and acceptance of case to the nursing and care coordination staff.
  • Adheres to University and unit-level policies and procedures and safeguards University assets.
