About The Position

The role of the Disability Service Navigator is to provide specialist coordination and advocacy for children, young people, and families whose needs cannot be effectively met through routine triage and referral, due to service or system factors and constraints. Service navigation requires deep service knowledge, active case facilitation, and authority to escalate across agencies when services are unavailable, inaccessible or fragmented. The Disability Service Navigator will work with children and families to ensure they do not remain ‘stuck’ in the system, falling between eligibility criteria or waiting lists, by actively coordinating pathways and addressing barriers.

Requirements

  • Current Tasmanian Registration to Work with Vulnerable People (RWVP) with a registration Status – Employment
  • A tertiary qualification approved by the relevant National Board, for example a Bachelor of Social Work, and eligibility for membership with the relevant National Board or professional association; or
  • An alternative tertiary qualification (AQF Level 7 Qualification) in a discipline considered relevant to the duties; or
  • An Associate Degree, Advanced Diploma or Diploma (AQF Level 5 or 6 Qualification) in a discipline considered considered relevant to the work, and relevant experience.
  • Conviction checks in the following areas: crimes of violence, sex related offences, serious drug offences, crimes involving dishonesty, serious traffic offences
  • Identification check
  • Disciplinary actions in previous employment check

Responsibilities

  • Provide specialist coordination and advocacy for children, young people, and families.
  • Actively coordinate pathways and address barriers for families.
  • Escalate across agencies when services are unavailable, inaccessible or fragmented.
