About The Position

Post-Acute Care Services provides interdisciplinary, patient-centered care focused on the quality of life, comfort, and support for patients and families after an acute illness, hospitalization, or are facing a serious illness. The leader of this department is responsible for the strategic, operational, and clinical oversight of core services including, social work, spiritual care (chaplaincy), volunteer services, hospice and palliative care admissions. The Director, Supportive Care Services provides comprehensive leadership and oversight for the coordination and delivery of Palliative Care and Hospice support services across the continuum of care. Ensures integration of psychosocial, spiritual, and supportive care services with clinical operations to deliver high quality, compliant, and compassionate care for both the patients who are recovering from a serious illness and those facing end of life care. This position is accountable for the direction and performance of social work services, chaplaincy, volunteer programs, and all palliative care and hospice admission functions. This includes ensuring timely and appropriate patient access, effective care transitions, and alignment with regulatory requirements and organizational goals. This position is responsible for the hospice referral process ensuring timely access to hospice services. In addition, the role is responsible for managing all aspects of the Hospice Volunteer Services and Bereavement Program. Professional relationships will be developed across the organization to establish result-producing relationships with the referral community including physicians, discharge planners, and community partners. The goal will be to establish affiliations with community organizations with the intent focused upon hospice growth and recruiting more volunteers to serve within hospice.
