About The Position
Great Lakes Healthcare Center, a member of the CommuniCare Family, is seeking a Director of Social Services (LSW). The role is responsible for the performance of all social and psychosocial functions and for supporting residents’ psychosocial well-being. The Director will collaborate with direct caregivers, counsel residents, family members, and staff, and advocate for residents to help ensure their needs are met. This is a first-shift position in Dyer, Indiana.
Requirements
- Experience with LOCs, care planning, and discharges.
- Four-year college degree in Social Work or a related field.
- Three to five years of prior work or life experience, preferably in a healthcare setting.
- Must be licensed through the State Social Worker and Counselor Board.

Responsibilities
- Oversee the performance of all social and psychosocial functions.
- Build and maintain effective working relationships with direct caregivers to encourage their participation in and support of programs and services that benefit residents’ psychosocial well-being.
- Counsel family members, residents, and/or staff members.
- Serve as a resident advocate and work with direct-care staff to ensure residents’ needs are met.
