About The Position
Great Lakes Healthcare Center, a member of the CommuniCare Family, is seeking a Director of Social Services (LSW). The role is responsible for the performance of all social and psychosocial functions and for supporting residents’ psychosocial well-being. The Director works with direct caregivers, counsels residents, family members, and/or staff, and advocates for residents while coordinating with care staff to help ensure their needs are met. This is a first-shift position in Dyer, Indiana.
Requirements
- A four-year college degree in Social Work or a related field.
- Three to five years of prior work or life experience, preferably in a healthcare setting.
- Experience with LOCs, care planning, and discharges.
- Must be licensed by the State.
- Must complete the licensure exam through the State Social Worker and Counselor Board.

Responsibilities
- Oversee the performance of all social and psychosocial functions.
- Develop and maintain good working relationships with direct caregivers to support their participation in programs and services that benefit residents’ psychosocial well-being.
- Counsel residents, family members, and/or staff members.
- Serve as residents’ advocate and work in harmony with direct care staff to help ensure residents’ needs are met.
