About The Position

The Director of Care Transition develops, coordinates, directs, and administers the facility’s marketing and public relations programs and services to support full occupancy. The role promotes the facility and its services to community organizations and referral sources, maintains knowledge of admissions requirements and facility programs, and communicates with prospective residents, families, and media. The position is located in the Denver Metro area.

Requirements

• 2–4 years of experience in marketing or public relations in a health care setting.
• Thorough knowledge of effective communication, mass media, advertising, and community relations principles as they relate to nursing facility operations.
• Ability to make direct sales calls and organize and deliver public presentations.
• Ability to establish, implement, and maintain an effective marketing and public relations program.

Nice To Haves

• Previous supervisory experience.
• Bachelor’s degree from an accredited college, specializing in marketing or communications, or equivalent.

Responsibilities

• Demonstrate commitment to the company’s mission, values, and standards of ethical behavior.
• Plan, develop, organize, implement, evaluate, and direct the facility’s marketing and public relations programs and activities to maintain and increase census and inform the community about the facility, its programs, services, and practices.
• Promote the facility and its services to outside sources, including physicians, discharge planners, community business members, churches, and schools providing adult education.
• Maintain thorough knowledge of facility admission requirements, services, programs, and admission policies and procedures.
• Conduct tours for community members and families, conduct pre-admission interviews with prospective residents or guarantors, and explain nursing facility services.
• Maintain ongoing communication with the media and contact them about new facility programs and promotions.
• Develop and regularly update the facility’s media contact file.
• Keep the Administrator informed of noteworthy newspaper, radio, and television coverage that may affect the facility or regulations governing it.
