About The Position

Diversicare is seeking a Director of Care Coordination to lead the alignment of care resources with patient needs, benefits, and service goals. The director oversees care coordination activities, the restorative nursing program, care planning, discharge planning, and documentation compliance. The role includes collaborating with clinical staff and communicating with patients and families about changes in condition and discharge plans.

Requirements

• Degree in nursing, therapy, or another related clinical field, such as social services, and a current license in the respective field.
• Knowledge of healthcare reimbursement, utilization management, and discharge planning.
• Experience providing strategic leadership, managing teams, and leading projects.
• Ability to formulate and implement short- and long-term strategic plans.
• Strong verbal, written, and interpersonal skills.
• Proven ability to lead a team to achieve desired outcomes.
• Strong communication and organizational skills.
• Knowledge of PDPM requirements of insurers.
• Understanding of regulatory requirements.

• Previous case management experience.

Responsibilities

• Facilitate daily care coordination meetings to align care resources with patient needs and benefits.
• Participate in and facilitate Engage/72 HR Admission meetings as part of the care management system.
• Manage the restorative nursing program and integrate it with the activity department.
• Communicate promptly with patients and families regarding changes in condition and discharge planning.
• Collaborate with the clinical team to develop and execute care plans.
• Coordinate discharge planning and post-discharge communication, including home health and durable medical equipment (DME).
• Monitor Quality Measures with the care coordination team.
• Ensure accurate and timely completion of skilled care documentation in accordance with Medicare and Managed Care regulatory guidelines.
• Review Additional Documentation Requests (ADRs) to ensure all requested documents are provided.
