About The Position

Diversicare is seeking a Director of Care Coordination to lead care coordination and help ensure care resources align with patient needs, benefits, and service standards. The role includes coordinating the restorative nursing program, facilitating daily care coordination and admission meetings, supporting care planning and discharge planning, and ensuring timely communication with patients and families. The Director also monitors quality measures and oversees accurate, timely skilled-care documentation in accordance with Medicare and Managed Care requirements.

Requirements

• Degree in nursing, therapy, or another related clinical field, such as social services.
• Current license in the respective clinical field.
• Knowledge of healthcare reimbursement, utilization management, and discharge planning.
• Experience providing strategic leadership, managing teams, and leading projects.
• Ability to formulate and implement short- and long-term strategic plans.
• Strong verbal, written, and interpersonal skills.
• Proven ability to lead a team to achieve desired outcomes.
• Strong communication and organizational skills.
• Knowledge of insurers’ PDPM requirements.
• Understanding of regulatory requirements.

Nice To Haves

• Previous case management experience.

Responsibilities

• Facilitate daily care coordination meetings to align care resources with patient needs and benefits.
• Participate in and facilitate Engage/72 HR Admission meetings as part of the care management system.
• Manage the restorative nursing program and integrate it with the activity department.
• Ensure timely communication with patients and families about changes in condition and discharge planning.
• Collaborate with the clinical team to develop and execute care plans.
• Coordinate discharge planning and post-discharge communication, including home health and durable medical equipment (DME).
• Monitor Quality Measures with the care coordination team.
• Ensure accurate and timely skilled-care documentation in accordance with Medicare and Managed Care regulatory guidelines.
• Review Additional Documentation Requests (ADRs) to ensure all requested documents are provided.
