About The Position

DaVita Integrated Kidney Care (IKC) is seeking a Director, Clinical Programs to lead high-priority care-model transformation and clinical-performance initiatives across its value-based care portfolio. The role converts clinical and business strategy into practical operations, leading complex initiatives from design through implementation, stabilization, and ongoing improvement while aligning clinical, operational, analytical, product, technology, and financial teams around shared outcomes. The initial portfolio includes care-model transformation for patients with chronic kidney disease and broader initiatives to improve clinical outcomes, reduce avoidable utilization, advance quality, and improve affordability. The portfolio may change as enterprise priorities evolve. The role offers an opportunity to shape value-based and risk-based kidney care models at scale, from strategy through day-to-day care delivery and measurable performance. The Director will lead and develop managers, analysts, and workstream leaders; establish accountability and effective performance systems; and build repeatable transformation capabilities. The position involves significant ownership, work with senior leaders, and the possibility of taking on broader enterprise scope as impact and business priorities evolve.

Requirements

  • Bachelor's degree in business, healthcare administration, public health, nursing, economics, operations, analytics, or a related field.
  • At least 8 years of relevant professional experience.
  • Experience in healthcare transformation, value-based care, population health, clinical operations, care delivery, quality, affordability, or a related field.
  • Demonstrated ownership of a complex healthcare initiative from strategy or design through implementation and operating adoption.
  • Experience leading cross-functional work in a matrixed organization.
  • Strong structured problem-solving, analytical, and communication skills.
  • Experience presenting recommendations, risks, and decisions to senior leaders.
  • Experience managing people, developing leaders, or leading multiple substantial workstreams.
  • Demonstrated effectiveness in complex and evolving environments.

  • Experience in value-based care, Medicare Advantage, population health, or complex chronic-condition management.
  • Experience launching, scaling, or transforming a healthcare delivery or care-management model.
  • Experience in an organization accountable for both clinical and financial outcomes.
  • Experience with clinical quality, patient engagement, Stars, HEDIS, or total cost of care.
  • Experience partnering with Analytics, Product, Technology, Finance, or Actuarial teams.
  • Healthcare consulting, enterprise transformation, or operating-model experience combined with meaningful implementation responsibility.
  • Graduate degree such as an MBA, MPH, MHA, MSN, or related advanced degree.
  • Clinical licensure and kidney-care experience are helpful but not required.

Responsibilities

  • Lead the design, implementation, and improvement of care models serving patients with complex chronic conditions.
  • Translate clinical and business priorities into operating models, workflows, roles, accountabilities, and management routines.
  • Identify performance barriers and coordinate teams to resolve them; develop approaches scalable across programs, populations, and markets.
  • Lead initiatives to improve patient engagement, clinical outcomes, quality, avoidable utilization, and total cost of care.
  • Partner with clinical and operational leaders to turn patient-risk and performance insights into focused interventions.
  • Evaluate whether operational activity is producing intended patient and business outcomes; use data and frontline insight to identify gaps and prioritize action.
  • Own complex initiatives from planning and readiness through implementation, stabilization, and sustainable operations.
  • Establish priorities, accountabilities, risks, and decision points, and ensure teams have the workflows, tools, training, reporting, and support needed to execute.
  • Resolve cross-functional issues through durable solutions rather than recurring workarounds.
  • Partner with Analytics, Product, and Technology teams to develop reporting and tools that support performance management.
  • Connect strategic goals with operational measures, clinical outcomes, and financial performance; translate complex performance information into clear insights and recommendations.
  • Lead matrixed work across Clinical, Operations, Analytics, Product, Technology, Patient Engagement, Finance, Actuarial, and other functions.
  • Build relationships with senior leaders and cross-functional partners, facilitate decisions across differing priorities, and communicate recommendations, tradeoffs, and required decisions clearly.
