About The Position
Learning Services, a Collage Rehabilitation Partner, is seeking a compassionate, dedicated, and proactive Direct Support Professional to support adults with brain injuries in a home-like neurorehabilitation setting. The role involves implementing rehabilitation strategies and providing guidance, support, and encouragement to help clients build independence, confidence, and progress toward personalized goals. Support includes assistance with daily living, vocational and recreational pursuits, health and wellness, community participation, and maintaining a safe living environment.
Requirements
- High school diploma or equivalent.
- Ability to lift up to 50 pounds.
- Valid driver’s license and a record of responsible driving behavior.
- Ability to travel and/or operate a vehicle for up to 20% of working time.
- Availability to work a consistent weekend day.
Responsibilities
- Assist, guide, and encourage clients with Activities of Daily Living, community living skills, and personalized rehabilitation goals.
- Assist with medication administration as assigned, monitor nutrition plans, and prepare well-balanced meals daily.
- Support clients through challenging situations and implement behavior management plans while maintaining professional boundaries and mutual trust.
- Observe and identify changes in clients’ behavioral, emotional, cognitive, or physical conditions, and alert the treatment team.
- Safely transport and accompany clients to medical appointments, group outings, errands, and recreational activities.
- Complete shift documentation, including daily data sheets, goal sheets, vital/seizure flow sheets, supervision checklists, and incident reports, in compliance with CARF, state, and federal standards.
- Maintain a clean, organized, safe, and welcoming living environment in alignment with licensing requirements.
- Participate in shift hand-offs and review operational communications, including emails, meeting minutes, and rounds summaries.
- Collaborate effectively with family members, care teams, and leadership.
