About The Position

ACHIEVE is hiring a Direct Support Professional to support individuals with moderate to severe disabilities in achieving independence, building friendships, and participating in community life. The role includes providing supervision, training, and support during volunteer and recreational outings, implementing individual goal plans, and recording participant progress. Training is paid and provided; no prior experience is necessary. The position is full-time, first shift, in Norwich, New York, with pay of $19.00–$20.25 per hour.

Requirements

High school diploma or GED.
Valid New York State driver’s license and a driving record acceptable to the agency’s insurance carrier.
Ability to lift and assist with up to 50 pounds, including transfers and physical support.

Responsibilities

Promote independence and support individuals with moderate to severe disabilities.
Transport individuals to community volunteer and recreational activities and provide necessary supervision, training, and support during outings.
Implement goal plans and collect data to monitor progress.
Maintain accurate daily participant records, charts, logs, and case or progress notes in accordance with the Direct Supervisor’s instructions and individual treatment plan methodologies.
Maintain an orderly, clean, and safe environment.
Order supplies with approval from the Direct Supervisor.
Assist individuals with toileting, if needed.
Complete monthly progress notes and assigned semi-annual and annual reviews, including in the absence of the Direct Supervisor.
Assist individuals with boarding and disembarking vehicles.
Monitor passengers and maintain order on vehicles to ensure safety and compliance with safety regulations.
Assist in planning activities that foster community integration.
