About The Position

Grace Community Care & Homes is developing its Developmental Disabilities and Autism department and seeks a Direct Support Professional Day Habilitation. The role contributes to person-centered services, reliable operations, and accurate documentation. The planned role is based in New Jersey, with administrative coordination through Grace and on-site or community assignments as applicable. The start date and duties are contingent on applicable program approvals, funding, credentialing, and staffing readiness. Services may begin only within approved locations and authorized scope. Weekly hours and the start date will be discussed during hiring.

Requirements

• Ability to complete required training and background screening.
• Ability to perform essential tasks with reasonable accommodation.
• Provide relevant credentials, experience, availability, and a resume.
• Final regulatory eligibility will be verified before appointment or service delivery.

Responsibilities

• Support communication, daily living skills, community participation, and individualized goals.
• Maintain participant safety, provide respectful supports, and complete accurate daily notes.
• Collaborate with department leadership, intake, credentialing, billing, and quality teams.
• Maintain confidentiality, respect participant choice, and promptly communicate service gaps.
• Follow role-specific policies and professional scope.
• Participate in orientation, competency review, and quality improvement.

Benefits

• Pay of $17–$21 per hour, based on qualifications and relevant experience.
