About The Position

The Direct Support Professional works in ACHIEVE’s Day Habilitation program in Norwich, NY, supporting individuals with moderate to severe disabilities. The role promotes independence, helps individuals discover their abilities and build friendships, and supports participation in community activities. Training is paid and provided, and prior experience is not necessary. This is a full-time, first-shift position paying $19.00–$20.25 per hour.

Requirements

High school diploma or GED.
Valid New York State driver’s license with a driving record acceptable to the agency’s insurance carrier.
Ability to lift and assist with up to 50 pounds, including transfers and physical support.

Responsibilities

Promote independence and provide support to individuals with moderate to severe disabilities.
Transport individuals to community volunteer and recreational activities, such as parks, shopping, and restaurants.
Provide supervision, training, and support during community outings.
Implement goal plans and collect data to monitor progress.
Maintain accurate daily participant records, charts, logs, and case or progress notes in accordance with supervisor instructions and individual treatment plan methodologies.
Maintain an orderly, clean, and safe environment.
Order supplies with approval from the Direct Supervisor.
Assist individuals with toileting when needed.
Complete monthly progress notes and semi-annual and annual reviews as assigned by, or in the absence of, the Direct Supervisor.
Assist individuals with boarding and disembarking from vehicles.
Monitor vehicle passengers and maintain order to ensure passenger safety and compliance with safety regulations.
Assist in planning activities that foster community integration.
