About The Position

Responsible for implementing habilitation and supportive living plans for participants as developed by the interdisciplinary team and ensuring a safe and healthy environment within the community setting. Position guarantees a minimum of 32 hours per work week, predicted on the employee being available to work anytime called. This is a safety-sensitive position. Maintains confidentiality of all information received regarding Friendship Community Care, Inc. and the clients. Conditions of confidentiality shall be in compliance with Friendship Community Care Non-Disclosure Policy and HIPAA Policies. Requirements

  • High school diploma or general education degree (GED).
  • One to three months related experience and/or training; or equivalent combination of education and experience.
  • Must have a valid driver's license.
  • Ability to read and comprehend simple instructions, short correspondence, and memos.
  • Ability to write simple correspondence.
  • Ability to effectively present information in one-on-one and small group situations to customers, clients, and other employees of the organization.
  • Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals.
  • Ability to compute rate, ratio, and percentage and to draw and interpret bar graphs.
  • Ability to apply commonsense understanding to carry out instructions furnished in written, oral, or diagram form.
  • Ability to deal with problems involving several concrete variables in standardized situations.
  • Must have a cell phone on hand when transporting consumers for emergency situations.
  • Required to undergo periodic driving test and demonstrate proficiency in the operation of a motor vehicle, including passenger vans.
  • Must have a clean MVR.

  • Employees may also be required to drive his/her own personal vehicle at times and maintain adequate liability coverage on any personal vehicle used to transport participants.

Responsibilities

  • Coordinates services for client.
  • Works closely with Service Manager to ensure good health care for clients is met, including proper nutrition, exercise, mental and physical health areas.
  • Coordinates with Service manager if concerning client needs/concerns.
  • Monitors all client medications to ensure proper guidelines followed, including documenting medications taken, counting medications, etc., as assigned by Coordinator or Service Manager.
  • Follows all financial procedures for client checking accounts and personal/grocery shopping, when supervising these activities.
  • Provides clients with transportation as needed for daily activities.
  • Complete all necessary paperwork for billing Medicaid transportation as allowable.
  • Conducts oneself in a professional manner.
  • Assists clients in personal hygiene, dressing, clothing maintenance, laundry, etc. as needed.
  • Assist clients in cooking meals as needed.
  • Complete all documentation forms as required by FCC, including Progress notes, Behavior reports and Marks reports.
  • Assists in maintaining facility/grounds in clean, orderly, and well-maintained fashion.
  • Assists in maintaining vans in good working order, including cleaning, and required preventive maintenance done per schedules.
  • Assists in maintaining log of all monthly emergency drills.
  • Attend training sessions, in/out of town, as required by FCC and licensing agency.
  • In case of emergency, take necessary steps and notify Service Manager, Regional Manager or appropriate person as soon as possible.
  • Serves as Driver for participant day trips to various state locations.
  • Employees may also be required to drive his/her own personal vehicle at times and maintain adequate liability coverage on any personal vehicle used to transport participants.
  • Adhere to and comply with all Quality Assurance’s & HIPAA procedures.
  • Follow the Code of Conduct and Compliance Investigation Procedures as read and discussed in the new employee training must be followed at all times.
  • Consumers’ progress notes be maintained and completed daily or as needed on the prescribed forms.
  • Consumers’ records must be maintained weekly, monthly, quarterly, and yearly.
  • All information must be filed in consumers’ records as needed and updated as prescribed by funding sources.
