About The Position

Assists with the daily activities and training of individuals present in the Social Rehabilitation Program. This role involves engaging with external referral sources, scheduling intakes, and coordinating with Case Management and Therapists. The position provides training and care in various activities designed to improve daily living skills, personal hygiene, functional and educational skills, pre-vocational skills, communication, personal adjustment, arts & crafts, social skills, physical development, recreational skills, community life, mobility, and integration/inclusion. It also involves sensory stimulation and gross/fine motor activities. The role requires assigning days of attendance based on consumer functional level, observing individuals and reporting changes to the Supervisor, and maintaining the cleanliness and order of the Program. The Direct Support Professional also assists in developing and updating service plans, maintaining Case Records, ordering and serving lunches, and managing the 'corner pocket' store to develop consumer money management skills. Adherence to all policies, laws, regulations, and codes of ethics, including confidentiality, is essential.

Requirements

  • Ability to receive, understand and communicate information with effectiveness and follow instructions.
  • Must successfully complete course requirements for certifications, as may be required by program.
  • Must successfully obtain Criminal History Clearance from the Pennsylvania State Police, Child Abuse Clearance from the Pennsylvania Department of Human Services and FBI Fingerprint Clearances.
  • Must have a valid Pennsylvania Drivers License and safe driving record.
  • Must not be excluded from participation in Medicare, Medicaid, or any other federal health care program.

Responsibilities

  • Engage with external referral sources, schedule intakes and coordinates with Case Management and Therapist.
  • Provides training and care in activities which include but are not limited to, the following: daily living skills, personal and hygiene needs, functional and educational skills, pre-vocational skills, communication, personal adjustment, arts & crafts, social skills, physical development and recreational skills, community life, mobility and integration/inclusion skills, and various sensory stimulation, gross and fine motor activities.
  • Assigns days of attendance based on consumer functional level.
  • Observes individuals and reports changes in behavior, attitude, or physical condition to Supervisor, in a timely manner.
  • Maintains the cleanliness and order of the Program and initiates emergency procedures as necessary.
  • Assists in developing and updating service plans, based on ongoing assessments, evaluations, observations, and interviews, maintains all related Case Records.
  • Orders and serves lunches.
  • Implements and maintains “corner pocket” store, purchasing items, handling petty cash for consumer operated store to develop money management and budgetary skills.
  • Adheres to all policies, laws, regulations and codes of ethics and confidentiality as outlined by federal and state laws, and agency policies and procedures.
