About The Position

The Clinical Coordinator is responsible for assisting with the management and operation of the clinic. This position assumes full responsibility for the dialysis clinic in the absence of the Facility Administrator.

Requirements

  • Current Registered Nurse (RN) license, in good standing, in the state of practice. License must be maintained as current and in good standing.
  • Current American Heart Association (AHA) Basic Life Support (BLS) Provider certification required upon hire and must be maintained without lapse throughout employment.
  • 12 months dialysis experience in a Charge Nurse or Registered Nurse position.
  • Confirmation of ability to distinguish all primary colors.
  • Demonstrated working knowledge of the English language and ability to communicate in writing as well as verbally.
  • Must have basic computer skills, including Microsoft Office (Word, Excel, Outlook); proficiency in all NRC clinical applications required within 30 days of hire.
  • Must successfully complete training program approved by the Medical Director, including demonstrated competency, within 30 days of hire.
  • Successful completion of clinical skills checklist within 30 days of hire and annually thereafter.
  • Knowledge of medical terminology and procedures related to dialysis.
  • Strong organizational and multitasking skills.
  • Excellent interpersonal and communication skills for effective customer interactions.
  • Proficiency in using electronic medical record (EMR) systems.
  • Ability to work in a fast-paced environment and remain composed under pressure.
  • Attention to detail to ensure accuracy and thoroughness of documentation and correspondences.
  • Excellent time management and task prioritization skills.

Responsibilities

  • Function as the hemodialysis team leader in the provision of chronic hemodialysis care and treatment. Provide day-to-day direction and supervision to assigned direct patient care staff.
  • Work with Facility Administrator on developing optimal staffing and patient schedules.
  • Oversee compliance with all CMS guidelines & organizational policies, and procedures. Functions as supervisor in the absence of Facility Administrator for all facility state and/or federal surveys.
  • Work with the Facility Administrator to maintain chronological, thorough, and appropriate documentation in the patient records.
  • Support Facility Administrator with staff education and in-service sessions as necessary.
  • Responsible for the orientation and training of new hire clinical staff.
  • Oversee clinical outcomes, communicate with the physician and other members of the healthcare team to interpret, adjust, and complete patient care plans.
  • Assist clinical staffing as needed.
  • Assist with clinical and operational procedures to improve patient health and minimize hospitalizations and missed treatments.
  • Demonstrate effective use of business supplies and efficient utilization of scheduled work hours.
  • Perform duties as assigned to meet the patient care and/or operational needs of the facility.
  • Document monthly review of patient medication profiles.
  • Administer in-center medications as ordered by the physician following the 5 rights of medication administration.
  • Transcribe orders from medical staff.
  • Assist with the implementation of anemia management and medication protocols per physician order.
  • Provide and document patient education and follow up as needed.
  • Complete patient admissions process as outlines in policy and procedure.
  • Complete pre and post treatment physical assessments.
  • Initiate and terminate patient treatments to include connecting and disconnecting catheters or vascular accesses and cleaning catheter/vascular access sites per policy and procedures.
  • Obtain blood samples, centrifuge samples and pack labs as required to complete physician orders.
  • Perform duties at all times under the supervision of the Facility Administrator and within limits established by and in accordance with company policy and procedures, applicable state and federal laws and regulations.
  • Comply with all applicable healthcare regulations, guidelines, and protocols.
  • Follow policies and procedures for personal protective equipment (PPE) use.
  • Maintain confidentiality of patient protected health information (PHI).
  • Maintain security of medical records.
  • Set up hemodialysis machines according to policies and procedures.
  • Respond to visual and audio alarms on machines; troubleshoot machine alarms.
  • Remove disposables and clean station after each treatment following policies and procedures.
  • Direct and lead the team in emergency response incidents.
  • Obtain pre and post treatment weight of patient per policies and procedures.
  • Safely initiate and terminate dialysis treatments per facility policies and procedures, and within scope of practice as allowed by state regulations.
  • Monitor all patients during dialysis treatments at a minimum of every 30 minutes or per policy and procedures with accurate documentation.
  • Assist with patient entry and exit within the facility as needed.
  • Review and approve treatment records ensuring accurate documentation of information related to patient treatment by end of shift.
  • Maintain supplies in the nurse’s station and medication preparation area. Assist in keeping supply room clean and organized.
  • Maintain workspace and unit common areas to ensure cleanliness and organization.
  • Supervise and confirm proper procedure of water testing at the beginning of each shift by the assigned staff member per policy and procedures. Document quality control logs throughout the day and after all treatments, as required.
  • Report any significant information or change in patient condition to the patient physician.
  • Report machine problems to the Biomedical Technician and Facility Administrator.
  • Participate in staff meetings as required. Attend in-services and continuing education sessions in compliance with company policy and procedures.
  • Complete patient rounds with providers.
  • Acts as vascular manager if there is none assigned yet otherwise works with vascular manager to ensure continuous progress and follow ups.
  • Works with Facility Administrator in QAPI preparation and reporting.
  • Assist Facility Administrator and team with CMS and/or State DHS survey plan of corrections.
  • Assist the Home Therapy department in program operations, trainings and clinical outcomes as applicable.
  • Always handle incoming and outgoing calls professionally and courteously, promptly answer calls while identifying oneself and the location.
  • Participate in team concepts and promote a team environment.
  • Follow all company policies and procedures.
  • Expected to report to work on scheduled day at scheduled time, working entire shift as per attendance policy.
  • All other duties as assigned.
