About The Position

Delaware Hospice is actively seeking a full-time Dementia Care Coordinator. This role plays a key part in supporting the team by providing care coordination, respite, education, and resources to individuals living with dementia, their families, and caregivers. The ideal candidate will be compassionate, detail-oriented, possess strong communication and organizational skills, and be able to multitask and prioritize independently. Requirements

Minimum of two (2) years of experience working in an environment utilizing basic medical practice knowledge
Dementia Specific Education for Center for Medicare and Medicaid Services regulations (to be done at time of orientation and will be supplied by Delaware Hospice)
Compassionate approach to supporting individuals, families, and caregivers navigating life-changing diagnoses
Strong interpersonal, communication, documentation, and organizational skills.
Comfort using care management systems, electronic health records, and other technology‑based tools.
