The Dementia Care Partner Program Coordinator serves as the primary point of contact for individuals and families enrolling in the Dementia Care Partners Program. This role is responsible for conducting comprehensive screening and enrollment of participants, administering standardized screening tools, and building trust through clear and effective telephone communication. The ideal candidate will excel at tracking and managing referrals efficiently, performing accurate data entry and creating encounters in the electronic medical record system, and communicating with providers regarding program referrals. Additional responsibilities include providing administrative support and coverage for the Associate Director, occasionally covering health coach visits during staff absences, and completing various administrative tasks to support program operations. This position provides a variety of program support functions which contribute to the overall management of clients’ healthcare, quality of life, and efficient use of resources. Additionally, this position provides a higher focus on administrative duties to ensure program requirement compliance. This position is responsible for communicating with eligible participants. Responsible for on-going client data collection and documentation in the electronic medical record, as well as communicating such data to the team to meet regulatory requirements. Responsible for completing chart reviews to determine program eligibility requirements to determine eligible cases for the program. This position is responsible for verbal communication with eligible participants and obtainment of data for program participation.
