About The Position

Banner Alzheimer’s Institute (BAI) is proud to offer research studies to patients and people in the community. Studies here are focused on Alzheimer’s disease and other dementias. As the Dementia Care Partner Program Coordinator, you will serve as the primary point of contact for individuals and families enrolling in our Dementia Care Partners Program. This role is responsible for conducting comprehensive screening and enrollment of participants, administering standardized screening tools, and building trust through clear and effective telephone communication. The ideal candidate will excel at tracking and managing referrals efficiently, performing accurate data entry and creating encounters in the electronic medical record system, and communicating with providers regarding program referrals. Additional responsibilities include providing administrative support and coverage for the Associate Director, occasionally covering health coach visits during staff absences, and completing various administrative tasks to support program operations. This position provides a variety of program support functions which contribute to the overall management of clients’ healthcare, quality of life, and efficient use of resources. Additionally, this position provides a higher focus on administrative duties to ensure program requirement compliance. This position is responsible for communicating with eligible participants. Responsible for on-going client data collection and documentation in the electronic medical record, as well as communicating such data to the team to meet regulatory requirements. Responsible for completing chart reviews to determine program eligibility requirements to determine eligible cases for the program. This position is responsible for verbal communication with eligible participants and obtainment of data for program participation.

Requirements

• High school diploma/GED or equivalent working knowledge and/or may be in training for a technical/trades career in the healthcare field.
• Moderate experience, typically gained through 1 year of relevant healthcare experience.
• Must have clinical knowledge in healthcare and working knowledge of medical terminology.
• Must be proficient with clinical care processes, clinical technology as well as use of the Electronic Medical Record.
• Must have highly developed critical thinking skills with the ability to prioritize and work autonomously as needed.
• Must be able to adequately convey messages and thoughts to a diverse population in both written and verbal formats.
• Must be able to maintain excellent organizational skills under the stress of deadlines.
• Must be computer literate and have proficient typing skills.
• Must possess the desire to work with the older population.

Nice To Haves

• Current and active Certified Nursing Assistant (CNA), Community Health Worker Certification (CHW), or Medical Assistant Certification in state of practice may be required in certain facilities and/or departments in accordance with regulatory requirements or facility/department policy.
• Motivational Interviewing skills, previous healthcare experience or experience in dementia care are preferred.
• Multilingual a plus.
• Additional related education and/or experience preferred.

Responsibilities

• Serve as the primary point of contact for individuals and families enrolling in the Dementia Care Partners Program.
• Conduct comprehensive screening and enrollment of participants.
• Administer standardized screening tools.
• Build trust through clear and effective telephone communication.

• Perform tasks required by program protocol, which may include obtaining patient medical history, systems review, and cognitive assessments on participants.
• Maintain quality assurance on designated protocols.
• Collect data as required.
• Ensure uniform implementation of protocols by completing required documentation in a consistent, accurate and timely manner.
• Complete work with direct supervision.
• May conduct visits with program participants.
• Provide educational training and information to patients and caregivers.
• May serve as a primary and direct contact person for patients and families.
• Actively participate in program evaluation and process improvement processes.
• Serve as a liaison to other departments, program partners as well as community agencies.
• May be involved in the training or onboarding of other program staff.
• Exhibit excellent interpersonal skills and a high level of customer service approach in all contacts with patients, caregivers, referral sources and staff.
• Respond to standard requests from internal customers.
• Interact primarily with department personnel, cross-department personnel, internal customers, patients, caregivers and physicians.
