About The Position

The patient care coordinator offers medical guidance, acts as a support group, and guides patients and their families through the diagnostic evaluation and treatment process. Provides education, support and resources to each patient, empowering them to make informed treatment decisions. Collaborates with other members of the interdisciplinary team to assess needs, develop care plans, maintain documentation, and serve patients and caregivers enrolled in Ochsner programs. Activities include, but are not limited to, education about the disease, medication review, advanced care planning, caregiver wellbeing, home safety, etc. Serves as the connecting agent between the patient/family, the healthcare system, and community resources.

Requirements

  • High School diploma or equivalent
  • 1 year of experience with direct patient contact
  • Effective verbal and written communication skills and ability to present information clearly and professionally to varying levels of individuals
  • Strong organizational, time management and prioritization skills and ability multi-task, pay close attention to detail, and learn and apply new techniques
  • Ability to work independently in a fast-paced environment and collaboratively in a multidisciplinary team
  • Proficiency using computers, software, and web-based applications including working knowledge of Epic
  • Ability to build rapport, empathize, support, and problem solve with individuals and families enrolled in the care management program

Responsibilities

  • Assists in recruiting and screening care program participants for eligibility on the telephone, in the clinic, and other settings as required
  • Coordinates and oversees patient care and activities as it relates to the program protocols and in accordance with Good Clinical Practice Guidelines, under direction and supervision of more senior staff
  • Remains a support system before and after the patient's treatment
  • Develops and maintains all required documentation as it relates to the program protocols, in compliance with best practices and relevant privacy laws and institutional policies
  • Monitors caseload tasks and needs; proactively escalates needs to appropriate clinical team members
  • Participates in team meetings, case reviews, and performance reviews
  • Performs other duties as assigned

  • Remains knowledgeable on current federal, state and local laws, accreditation standards or regulatory agency requirements that apply to the assigned area of responsibility and ensures compliance with all such laws, regulations and standards.
