About The Position

Delaware Hospice is actively seeking a full-time Dementia Care Navigator. The Dementia Care Navigator is a key member of the Delaware Hospice Dementia Care GUIDE Program, serving as the primary non-clinical point of contact for individuals living with dementia and their caregivers. The role provides relationship-based caregiver support, care coordination, and navigation services using standardized workflows and technology-enabled tools, in alignment with the CMS Guiding an Improved Dementia Experience (GUIDE) Model. The Care Navigator works collaboratively with the interdisciplinary team members to support high-quality, consistent dementia care delivery, caregiver engagement, and program scalability. This is an exempt position.

Requirements

Minimum of two (2) years of experience in care coordination, patient navigation, case management, or healthcare‑related service delivery.
Strong interpersonal, communication, documentation, and organizational skills.
Comfort using care management systems, electronic health records, and other technology‑based tools.
Dementia Care certification/training required (provided through Delaware Hospice).
Valid driver’s license required.
Ability to work remotely in a hybrid model with periodic in‑person or community‑based engagement as required.

Nice To Haves

Experience working with older adults, individuals with dementia, and/or family caregivers preferred.
