About The Position

Delaware Hospice is actively seeking a full-time Dementia Care Navigator. The Dementia Care Navigator is a key member of the Delaware Hospice Dementia Care GUIDE Program, serving as the primary non-clinical point of contact for individuals living with dementia and their caregivers. The role provides relationship-based caregiver support, care coordination, and navigation services using standardized workflows and technology-enabled tools, in alignment with the CMS Guiding an Improved Dementia Experience (GUIDE) Model. The Care Navigator works collaboratively with the interdisciplinary team members to support high-quality, consistent dementia care delivery, caregiver engagement, and program scalability. This is an exempt position.

Requirements

  • Minimum of two (2) years of experience in care coordination, patient navigation, case management, or healthcare‑related service delivery.
  • Strong interpersonal, communication, documentation, and organizational skills.
  • Comfort using care management systems, electronic health records, and other technology‑based tools.
  • Dementia Care certification/training required (provided through Delaware Hospice).
  • Valid driver’s license required.
  • Ability to work remotely in a hybrid model with periodic in‑person or community‑based engagement as required.

Nice To Haves

  • Experience working with older adults, individuals with dementia, and/or family caregivers preferred.
