About The Position

American Senior Communities is hiring a Licensed Practical Nurse for dementia and memory care at Washington Healthcare Center. In this role, you will build meaningful relationships with residents and staff by learning their life stories. You will use your nursing skills to promote residents’ physical and psychosocial well-being and contribute to a comforting, engaging atmosphere and coordinated care.

Requirements

• Current, valid Licensed Practical Nurse license in Indiana.
• Experience in dementia or memory care.
• Proficient medication management skills.
• Ability to conduct thorough assessments and accurately document changes in resident condition.
• Passion for geriatric nursing and commitment to senior care excellence.
• Excellent communication and interpersonal skills.
• Demonstrate the C.A.R.E. values: compassion, accountability, relationships, and excellence.

Responsibilities

• Create a comforting and engaging atmosphere for residents.
• Promote teamwork within the care team to meet residents’ needs.
• Collaborate with nursing and other facility teams to provide coordinated, comprehensive care.
• Work toward the shared goal of excellent resident care.
• Support a respectful and positive work environment.
• Manage medications proficiently.
• Conduct thorough assessments and accurately document changes in residents’ conditions.
