About The Position

We are hiring for: Deaf/Hard of Hearing Residental Care Coordinator Type: Regular If you are a positive and personable individual looking for a satisfying and fun opportunity to make a real difference in the lives of people with intellectual, developmental disabilities, and people facing mental health, and substance use challenges, join our team at RHA Health Services! The Resident Care Coordinator is responsible for coordinating client care related to scheduling appointments, transportation, insurance, and communication with the care team. In addition, this position guides and assists individuals, that are deaf/hard of hearing and may have additional disabilities, to live and function as independently as possible in the community and participates in the development and implementation of individual program plans and coordination for the provision of specialized services. The Resident Care Coordinator must be fluent in American Sign Language.

Requirements

  • MUST BE FLUENT IN AMERICAN SIGN LANGUAGE (ASL)
  • High School diploma or equivalent required
  • At least 2 years working in a residential setting required
  • Proficiency in Microsoft applications
  • American Sign Language is required

Nice To Haves

  • Bachelor’s degree in Human Service Field preferred

Responsibilities

  • Coordinate and schedule medical, psychiatric and dental/vision appointments with doctors’ offices, health care providers and laboratories
  • Communicate and liaise with physicians, nurses and other relevant behavioral health and health care professionals regarding the medical needs of the resident
  • Document all physical, health care, dental, vision appointments
  • Prepare and complete all necessary documentation in accordance with program standards within required time frames as required
  • Perform Direct Support Services when needed
  • Travel between sites on a regular basis
  • Other duties as assigned
