About The Position

The Critical Care/ED Pharmacist attends all hospital codes and priority alerts, facilitates emergency medication provision, and works to provide comprehensive pharmacy services for all patients of the hospital and ED. This role involves attending Code Blues, Rapid Sequence Intubations, Code Strokes, and Code Sepsis, providing necessary medications, and making treatment recommendations. The pharmacist will also review and provide comprehensive pharmacy services for all STAT orders in the ED and ICU, monitor the EPIC verification queue, perform drug utilization reviews, monitor for drug interactions, assist in maintaining formulary integrity, participate in antibiotic stewardship, provide pharmacokinetic dosing and monitoring, and evaluate potential adverse drug events. The role functions as a clinical resource for hospital staff and ensures completion and accuracy of required documentation and record keeping. Requirements

• Graduate of accredited college of pharmacy.
• 1 year American Society of Health-System Pharmacist (ASHP) accredited residency or 4 plus years of acute hospital experience working in patient care area of specialty that demonstrates direct patient care in medication management and participation in patient rounds with medical and hospital staff.
• Pharmacy license from Texas State Board of Pharmacy or eligible for same.
• Specialty Board Certification or complete within 24 month of hire.
• Current JPS recognized Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) and Pediatric Advanced Life Support (PALS) certification and must be maintained throughout employment. May complete within 3 months of hire.
• Preceptor with State Board of Pharmacy or complete within 6 months of hire.

Nice To Haves

• 2 year American Society of Health-System Pharmacist (ASHP) accredited residency in critical care or emergency medicine.
• National IV Certification

Responsibilities

• Attends Code Blues including traumatic arrests, provides necessary medications, and documents participation.
• Attends Rapid Sequence Intubations in all areas of hospital and provides necessary medications.
• Attends Code Strokes, reviews patient profile, makes recommendations for treatment, and mixes fibrinolytics if warranted.
• Attends Code Sepsis, reviews patient profile, makes antibiotic recommendations, and mixes necessary medications.
• Reviews and provides comprehensive pharmacy services for all STAT orders in the ED and ICU.
• Monitors EPIC verification queue for all STAT orders and those requiring clinical intervention or iVents.
• Performs drug utilization reviews, monitors for drug interactions, assists in maintaining integrity of formulary, participates in antibiotic stewardship program, provides pharmacokinetic dosing and monitoring services to the medical staff and evaluates potential adverse drug events.
• Functions as a clinical resource for the hospital staff.
• Ensures completion and accuracy of required documentation and record keeping.
