About The Position

Recovery Network of Programs, Inc. (RNP) is a private, non-profit, social service agency serving the Greater Bridgeport Community and Fairfield County since 1972. RNP is dedicated to helping individuals with substance use disorders, mental health disorders, and homelessness, aiming to restore hope, health, and wellbeing. The Prospect House program provides emergency shelter for thirty-six adults daily in Bridgeport, CT, offering comprehensive case management services to address the complex needs of homeless individuals and connect them to long-term housing solutions. Case management includes assessment, transportation, life skills training, and individual, group, and family sessions.

Requirements

  • Bachelor's degree in the behavioral sciences and two (2) years experience in counseling preferred; or equivalent combination of education and experience.
  • Co-occurring competencies and or credentials are preferred.
  • Knowledge of principles and techniques of social service case management services.
  • Knowledge of principles and techniques of basic case management, trauma informed care, motivational interviewing, crisis intervention, problem solving and de-escalation techniques.
  • Knowledge and understanding of the physical, psychological, social and economic effects of substance abuse, homelessness, physical and emotional illness, crisis intervention.
  • Ability to gather, analyze and evaluate information pertinent to client care.
  • Ability to coordinate multiple tasks simultaneously.
  • Knowledge of Housing and community service resources.
  • Ability to facilitate groups.
  • Considerable knowledge of federal and state laws and regulations regarding client confidentiality.
  • Able to work with a team approach.
  • Ability to establish and maintain cooperative professional relationships.
  • Excellent communication skills.
  • Ability to project a professional image.
  • Valid Connecticut driver's license is required.

Responsibilities

  • Assesses and records medical, psychological, and social factors contributing to client's situation and evaluates these capacities.
  • Assertively links individuals to community resources and other natural supports.
  • Develops and documents person-centered recovery plans.
  • Maintains timely progress notes which record and monitor client progress with regard to finances, employment, nutrition, basic needs, housing, physical health, mental health and substance abuse related issues.
  • Serves as an advocate for clients in obtaining necessary services.
  • Adheres to all standards as it relates to documentation requirements.
  • Completes and submits documentation as required to the Special Populations Department.
  • Attends and is prepared for case management and staff meetings.
  • Prepares weekly supervision agenda.
  • Maintains work schedule.
  • Assumes all other appropriate duties as deemed necessary by the supervisor.
