About The Position

Transition Home Healthcare is an elite home health agency serving Houston, TX and surrounding areas since 2010. We are a group of passionate people dedicated to delivering the highest quality services to our patients, families, team members, and the community. Our team is growing and looking for a dedicated: Certified Occupational Therapist Assistant (COTA) - Home Health. Service Area for Coverage: Southwest & Southeast.

Requirements

Certified Occupational Therapist Assistant in the State of Texas
Knowledge of Home Health regulations and general healthcare compliance
Ability to handle physical workload. Good driving record.

Nice To Haves

Bilingual (Fluent in Spanish) preferred
Home Health experience highly preferred

Responsibilities

Facilitation of rehabilitation of patients with emotional, physical or mental disabilities
Implementation of occupational therapy care plans
Coordination of care with entire care team
Educating patients and family members
Preparing timely, complete and accurate clinical notes
Communicating effectively with patients, caregivers, healthcare providers, office support and other care team members
