About The Position
Transition Home Healthcare is an elite home health agency serving Houston, TX and surrounding areas since 2010. We are a group of passionate people dedicated to delivering the highest quality services to our patients, families, team members, and the community. Our team is growing and looking for a dedicated Certified Occupational Therapist Assistant (COTA) - Home Health.
Requirements
- Certified Occupational Therapist Assistant in the State of Texas
- Ability to handle physical workload.
- Good driving record.
Nice To Haves
- Bilingual (Fluent in Spanish) preferred
- Home Health experience highly preferred
- Knowledge of Home Health regulations and general healthcare compliance
Responsibilities
- Facilitation of rehabilitation of patients with emotional, physical or mental disabilities
- Implementation of occupational therapy care plans
- Coordination of care with entire care team
- Educating patients and family members
- Preparing timely, complete and accurate clinical notes
- Communicating effectively with patients, caregivers, healthcare providers, office support and other care team members
