The Coordinator role is responsible for maintaining an active caseload of individuals referred for START Services. This involves providing systemic consultation based on assessments and outreach findings, utilizing ecomaps, and consulting with systems/teams using best practices. The role requires regular outreach visits to individuals' homes, day programs, or vocational settings for evaluations and assessments, conducted in-person or via telehealth as needed. The Coordinator will assist with referrals for consultation and treatment, including screening for START enrollment and completing intake assessments. They will coordinate support meetings and crisis plan development, using the START Action Plan and escalating concerns to leadership as appropriate. The position involves developing various service evaluations and crisis prevention plans, participating in consultations and meetings with START leadership and the clinical team, and providing on-call support on a rotational basis. Accurate and timely documentation, data entry into the START Information Reporting System, and maintaining community partner linkages are essential. The role also includes developing materials, presenting at meetings, and participating in the Center for START Services National Network. Compliance with all training requirements, YAI policies, and regulatory agency requirements (HIPAA, NCSS, OPWDD, DDS) is mandatory, including obtaining START Coordinator certification. The Coordinator must also comply with all Federal, State, and Local regulatory agency requirements.
